Data Request Form

Description and Policies

Appropriate use of this form includes requests for Rhode Island Student Survey (RISS) or Rhode Island Young Adult Survey (RIYAS) data in either aggregate tables or as line-item data sets that are de-identified.

All requests have a minimum turnaround time of five (5) business days. After the requested data has been sent to the requesting organization, the requestor will have one (1) week to review the data and request any additional data or modifications. After one (1) week has passed, the data request is closed. If additional data is needed, a new data request form must be submitted.

Please note: If you are publishing any reports, papers, one-pagers, or other material using BHDDH data, including web postings, you are required to cite BHDDH. If you are publishing an article in a peer-reviewed journal you are required to include a BHDDH employee as a co-author.

Default format is Excel. Data can be entered in a word document or saved as a PDF. Larger files may require a CSV.
 

Appendix A: BHDDH’s Small Cell Size Suppression Policy Agreement

If you are requesting de-identified line-item data, please read BHDDH’s Small Cell Size Suppression Policy and confirm your understanding of the policy by signing below. All individuals who will have access to this data set must sign this document.

BHDDH’s Small Cell Size Suppression Policy

“Reports should not contain any level of detail that can make individual identities distinct or discernible. In order to ensure that client privacy is protected in all BHDDH Data Unit reporting, any cells less than 5 reported externally from BHDDH should be suppressed. When suppressing cells for small size, please include the language ‘suppressed due to small cell size’ with an asterisk. Another way to approach the small cell issue is to regroup (i.e. if counts of American Native and Hawaiian Pacific Islanders served in an area are below 5, combining them into an ‘other race’ category may alleviate the need to suppress the cells). BHDDH retains discretion about application of the Small Cell Size Suppression Policy.”

I hereby understand, acknowledge, and agree that I am not permitted to share data, tables, reports, charts, presentations, one-pagers, data briefs, or data presented in any other format that include cell sizes smaller than five (5) or that include enough information for someone to calculate a cell size smaller than five (5). I understand, acknowledge, and agree that I am not permitted to share this line-item data set with anyone who has not agreed to this policy by signing this document.
